Added specific reimbursement rates for care evaluations, set at 300 percent of the medical assistance rate for a skilled nursing visit.

Maintained the effective date of January 1, 2026, or upon federal approval, whichever is later, for the new care evaluation coverage and rate increases.

Added a new definition for 'care evaluation' as a face-to-face evaluation to develop, update, or review a recipient's plan of care for home care services.
