HB 4106 requires pharmacists in Texas hospitals with more than 100 beds to create accurate medication profiles for high-risk patients admitted to those facilities, with "high-risk" defined by each hospital's internal policy. Trained pharmacist-interns or pharmacy technicians may complete these profiles if they meet state board-approved training requirements. Hospital pharmacists-in-charge must develop policies for implementing this process, including timelines for profile completion. The state board will later establish detailed rules about required profile contents and training standards. The law takes effect September 1, 2025.
